Form to dismiss a semester I couldn't attend for health?

I think you should try calling a few more offices, and tell the student answering the phone that you need to be directed to a professional to answer your question. It is best to call early in the morning (before lunch) as the individuals you need to speak to will be available. First, call the Registrar and find out what exactly happened with your classes, and if you can drop/withdraw from them now. Also have them send you a transcript so you can see for yourself. Call Academic Advising/Academic Services and make a phone appointment with your advisor and see if they are aware of the form and your situation. Give Financial Aid another try, and insist on speaking to a Financial Aid Counselor or the Director for help. Tell them you will make a phone appointment if necessary since you do not live close enough. I would also go ahead and send your professors whose classes you were in to see if they were aware of your situation and could possible point you in the right direction. Be sure that you also have all of the documentation of your situation (ss you have said, police reports, doctor's notes etc) so you are ready with this information and won't have to wait for that.
